System wide. A access date has to be updated in the directory of every file that is read (files that are written to already have their directory entries updated, but many more file are open for read access than for write).

Date accessed is not updated by default in Windows 7,8 and 10 (and Vista?) for performance reasons. The field gets whatever value it has from file creation and a few other things, but not normal access. Also 7-zip (file compression program) and Norton anti-virus are missing in some context menus through Everything (.gz files, for example). Maybe what these programs have in common is that they use dynamic context menus (with a .dll file) instead of fixed entries in the registry.

Everything does not index content, but it can search for it. It is slow if you have many files, but quite reasonable if you can limit the size of the search:

The short answer is no. There is no good way of telling what changed the date of a directory entry. Yes they sometimes do. For example, if I fat-fingered part of a file name , say "financial" it will not show up when I search for all of my "financial" files. Everything has many features I do not use, but are useful to others.

Tried to preview pdf files with 1.4.1.885. Everything hangs (not a cpu loop); After canceling & restarting, Search options have been changed from Match Path to Match Case ! Have a debug file, if you want it.

Tools>Options>General>UI uncheck Run in Background There is overhead in writing out the database each time Everything ends, extra writes to the SDD, overhead in starting the process, etc. Unless you are a very infrequent user or your system is constrained it might just be better to leave it running.

Under Indexes=>NTFS for each disk, do you have Include in database, Enable USN Journal & Monitor Changes checked?

When you reinstalled Everything, did you delete all copies of Everything.ini first ?

Try having BG exit between steps 4 and 5. I believe the database on disk is only written when Everything exits. The is also a file permission conflict if BG is writing the database when FG has it open for read

That would assume that the only location for everything.exe is C:\everything.

That is not the case. The shortcut would depend on the location chosen by the user and thus could not be packaged in the .zip.
